My buddy has a twin truck to mine. 15 CC SV. Horrible noise behind the dash confirmed to be a bad actuator. I was able to scan it while it happens and can see that the "Passenger Mix Feedback" is stuck around 4.3 (open). What I can't tell, is which actuator is the problem. This is the diagram from Nissan. I think it's the top actuator, but its echoing through the whole housing so it's hard to say. It looks like it's going to be a pain regardless... If someone can confirm which actuator is the passenger mix/temp control and where it is on this diagram - it would be greatly appreciated.

It's the center bottom one. You should be able to access it, it's above the passengers left foot on the bottom of the hvac plenum.
Just an FYI, actuators usually aren't very noisy. Keep your fingers crossed that it's not the whole door that is dislodged or broken.
Thank you - I found it in the factory repair manual before I saw your reply:

Better than it being the top one!!! I will pull the old one out and get one on order. Thanks!

Is there an alignment procedure in the repair manual for the motor? I know on a GM, you pull a fuse for a specified time, put the temp control in a certain position and then reinstall the fuse. You do something else that I forgot but then you wait a few minutes (specified in the procedure) and the motor should then be aligned. For GM's, that is often all that is needed.

This procedure has to be done on GM vehicles anytime you replace the actuator so it should be under the install procedures in the FSM.

BTW, they are not hard to take apart and fix. Usually they just jumped a gear or two.

No coding/reset that I can see. I'm using a Xtool D7 for diagnosis that has come in very handy!!! If there is an adaptation, it should be able to do it, but I'm thinking it's not necessary based on the procedure:

FRONT AIR MIX DOOR MOTOR - PASSENGER - ATC
Removal
1. Remove the front heater and cooling unit assembly. Refer to VTL-14, "Removal and Installation".
2. Remove the steering member from the front heater and cooling unit assembly.
3. Disconnect the harness connector from air mix door motor.
4. Remove the three screws and the air mix door motor.
Installation
Installation is in the reverse order of removal.

Couldn't get an ETA from Nissan on an OEM part - ordered a Dorman off Amazon. Figured for the bottom I could swap it easy enough if it fails quickly. But.... the bottom is not actually the passenger side. It's the driver's. I verified by watching the feedback while the actuator was disconnected. Drivers side cycled max/min while the actuator spun. Passenger barely moves while making grinding noises. Time to dig deeper. I'll put the good OEM part on top and leave the Dorman on the bottom.

When you say "upper actuator" are you referring to the "intake door motor?" Mine is popping, and for now it's disconnected so I can't switch between fresh/recirculate. If I can get to it by removing the passenger airbag I may be willing to do that. Removing the entire HVAC, nope...

I attempted to reply from my phone a couple weeks ago but I guess it didn't work! I pulled the passenger airbag and was able to reach through to change the upper actuator fairly easily. It was a chinese replacement held in with one screw. No signs otherwise that someone had been in there. I was able to move the flapper easily with my fingertips and felt no resistance. I put the used OEM actuator on the top and left the Dorman on the bottom. Nice and quiet!!! Temperature control works perfectly now.
