43k on an 03 is good due to i have 43k on my 04, a nissan tech told me 30k the front brakes start to weat, (mine wore at 27k), we have a timing belt and that should be changed at 100k along with a new water pump and so forth, other than that just keep all fluids topped off and changed when needed, me im due for a tranny fluid changed as well as my differentails, way over due, also a can of seafoam and injector cleaner is also a good idea just to keep the carbon build up low, as clean engine is a happy engine.

I woudl say change the transmission fluid. I did not have to do a break job untill about 50,000 mile or so but there is nothing major at the mileage you are at

for tranny oil, is flush necessary? would drain and refill be ok?

Slight rattle at cold start seems to be valve lifters... there's been a couple threads floating around here on that; just search titles for "rattle" and "startup" and you should find it.
Seafoam is like an uber-detergent you use to flush your whole system of excess (carbon? I believe?) and other buildups the normal detergents in oils and fluids don't remove. It's pretty useful stuff apparently; you can get it at any auto parts store. I haven't tried it yet but there's a really good thread on that too... again, search for "seafoam" and you should find it. (Sorry I'm not linking for you, I'm just being lazy right now.)

What I do is drain the transmission fluid and pull the pan and change the filter make sure and wipe down the pan and clean it out. It will have some metal shaving in it but that is normal and refill with about 6 quarts I do this about every 50,000 miles and had no problems
